About the Report Card History Table
The Facility Search Results table lists the nursing homes that meet the search criteria you provided on the search tab. Please take note of these features of the Results table:
  • The number of facilities found, along with your search criteria, gives you a quick summary of the search results.
  • Sorted Alphabetically or Ranked by Priorities indicates the order in which the facility list is presented. If you want to change the way your list is sorted, click the SEARCH tab, change the priorities, and click the "Click to See Your List" button.
  • Show underlying numbers button Report Card scores are expressed by the number of stars shown. Each star rating represents a range of performance. To see the exact score, click this icon.
  • Create Excel File button Click this button to create an Excel file that you can save on your computer.
  • The Print button allows you to print the table, without or without the background numbers, depending on what is showing at the time.
  • The Minnesota Average for each quality measure is shown in green. It is the top row in the table when the list is sorted alphabetically. But when you select one or more priorities, the Minnesota Average is sorted along with the nursing homes. This indicates which facilties are above average in the measures you have chosen as priorities.
The seven quality measures are described below.
  • Minnesota Clinical Quality Indicators - (Last Updated on 1/11/2019 using 10/1/2017-9/30/2018 data.)
    This is a measure of clinical quality of care derived from a comprehensive assessment of every nursing facility resident called the Minimum Data Set (MDS).
    Updates: MN Quality Indicators information is updated quarterly, for all facilities at the same time.

  • Resident Quality of Life - (Last Updated on 4/2/2018 using 2017 survey data.)
    This measure is based on face-to-face interviews of residents in each nursing facility in the state.
    Updates: Resident Quality of Life information is updated once yearly, for all facilities at the same time.

  • Family Satisfaction Survey - (Last Updated on 4/2/2018 using 2017 survey data.)
    This measure is based on mailed surveys of the primary person responsible for each nursing facility resident in the state.
    Updates: Family Satisfaction information is updated once yearly, for all facilities at the same time.

  • State inspection results - (Last Updated on 11/19/2018.)
    Each facility is inspected at least once a year to ensure that they meet federal quality standards to participate in the Medicare and Medicaid programs. The inspection findings are evaluated to determine this score.
    Updates: State Inspection Results information is updated quarterly for those facilities that have had an annual or other inspection during that quarter.

  • Hours of direct care - (Last Updated on 1/11/2019 using 10/01/2016-9/30/2017 report year data.)
    This is the average hours per day of all staff that provide direct care to residents (nurses, nursing assistants, medication aides, mental health workers, social workers, activities staff, and other direct care staff). To fairly compare facilities, the number is weighted depending on each facility’s types of staff and level of care provided.

  • Staff retention - (Last Updated on 1/11/2019 using 10/01/2016-9/30/2017 report year data.)
    This is the percent of direct care employees who have been in their jobs for at least one year.
    Updates: Staff Retention information is updated once yearly, for all facilities at the same time.

  • Temporary staffing agency use - (Last Updated on 1/11/2019 using 10/01/2016-9/30/2017 report year data.)
    This is the percent of direct care hours provided by an outside, temporary worker agency, rather than by staff employed by the facility.
    Updates: Temp Staff Agency Use information is updated once yearly, for all facilities at the same time.

  • Proportion of single rooms - (Last Updated on 1/11/2019.)
    This is the percent of the beds in the facility that are in single bed rooms.
    Updates: Proportion of Beds in Single Bed Rooms information is updated quarterly, for all facilities at the same time.
